Publications, Video, Internet Consent Opt-Out Form
Students who attend the Warwick Public School District are occasionally asked to be a part of school and/or District publicity, publications and/or public relations activities. If you do not wish to have your student’s image published, the District asks that you fill out and submit this form, which will be sent to administration and your student's school administration. If you have more than one student, please submit one form for each student in your household.
Your student should know this number, it's used for lunches and many other logins. It can also be found in the Family Portal -
You will receive a confirmation email containing the details of this form for your records.
Your signature below indicates that you DO NOT consent to your student's name, picture, art, written work, voice, verbal statements or portraits (video or still) appearing in school publicity or District publications, videos or on the District's website. Examples would include local television stories on school events, videos and print material produced by district staff, pictures and videos of student activities posted on school websites, school newsletters, and school social media pages, and photographs in local print publications.
Check the box above if you wish to have your student opted out of all district publications EXCEPT the school yearbook.
